Responsibilities include conducting comprehensive health assessments, reviewing medical history, medication adherence, social factors, and collaborating with physicians, social workers, and caregivers.

Assessments are performed in homes, skilled facilities, and community settings, lasting up to one hour.

Qualifications include a Master's Degree or equivalent, current RN and NP licensure with prescriptive authority, board certification, CPR/BLS, and at least 1 year of NP experience.

Flexible hours including days, evenings, weekends, with competitive pay ($2,300-$11,500/month based on completed assessments).
