Seeking a full-time Licensed Practical Nurse supporting primary care services at a community health center.

• Provide nursing care, including patient interviews, vital signs, health history, and medication updates.
• Perform in-house labs, collect specimens, and ensure quality control.
• Document patient information, follow up on labs and encounters, and assist with health education.
• Support clinical staff under supervision, without independent triage or data interpretation.
• Qualifications include a high school diploma, valid NY LPN license, CPR, and infection control certification.
• Preferred experience is 2-4 years; entry-level candidates with 0-1 year are also considered.
