The Home Health RN Care Manager coordinates and manages patient care in the home setting, utilizing advanced assessment, teaching, and decision-making skills.

• Provides comprehensive evaluations, develops individualized care plans, and educates patients, families, and caregivers.
• Ensures care aligns with physician orders, regulatory standards, and quality measures.
• Delegates tasks to team members and supervises care to promote safety and continuity.
• Requires a valid RN license, minimum one year of clinical experience, and BLS certification.
• Preferred experience includes Medicare-certified home health case management and a bachelor's degree in nursing.
